This guide helps you think through all aspects of your health — what’s going well, what could be better, and your questions or concerns. Being prepared means that you can make sure your appointment focuses on the things that matter most to you.

  • To view the guide on a Windows computer:
    • Open the guide
    • Click the “zoom in” button (the “+” icon) to make the text as large as you like
  • To print the guide on a Windows computer:
    • Open the guide and press print
    • On the print screen, click on “Fit” and the printed guide will expand to fill the 8.5” x 11” page
  • To view the guide on a Mac computer:
    • Open the guide
