Even with gains achieved to date, health care needs a much more proactive and prospective approach to improving safety. There is already evidence and precedent from other industries, such as aviation, of what health care systems may require to move toward breakthrough performance in safety and quality.
This article describes how data-driven stakeholder cooperation can help produce proactive solutions to improve safety for patients and the health care workforce. "Radical collaboration" builds and leverages new relationships among stakeholders, centers patients and their families in the work, engages every part of the health system, and uses emerging technologies — especially artificial intelligence (AI) — to both support and augment the efforts of stakeholders.
