Deliver the Right Care, in the Right Setting, at the Right Time

In the post-pandemic era, safe, equitable, and high-quality care takes on a sharper focus for hospitals and clinicians like never before. When faced with these inputs, managing hospital flow to meet the care needs of communities takes on the utmost importance. These goals can be impacted by: increasing patient complexity and acuity; higher degree of frailty; highly complex care processes; design layout of aging facilities; and staffing challenges.

Learning Goals
Learning Goals

Ensuring timely patient care in the right location at the right time with the right clinical team is critical.

Through this course, participants will learn how to adopt and apply core concepts for governing complex systems to achieve hospital-wide patient flow: Right Care, Right Place; Right Time; and Available Capacity. These core concepts form the basis of a multidisciplinary flow philosophy that unites all staff and departments to a common purpose.

After completing the course, participants will have skills to apply these concepts at an organizational level to prioritize high-level strategies to improve hospital-wide flow. Constructing an action plan to focus improvement efforts within a multidisciplinary team context, they will be able to lead the following activities, among others, at their individual organizations:

  • Generation of ideas to help build or strengthen the will for achieving hospital-wide flow (skills)
  • Creation of a hospital-wide learning system to understand failure, focusing on the development of approaches to mitigate flow failures and flow delays (strategy)
  • Articulation where investment is needed for successful execution of hospital-wide change initiatives to improve hospital-wide performance (performance)

The High Stakes of Suboptimal Flow

Optimizing patient flow throughout the hospital is essential to ensuring safe, high-quality, patient-centered care and capacity for growing and aging populations. Providing timely access to appropriate sites of care and optimizing hospital flow are critical levers that increase value for patients, clinicians, and health care systems. Patient and clinician experience is strongly influenced by the efficiency and effectiveness of care reflective of hospital flow.

Poorly managed flow has critical implications, such as:

  • Delaying treatments
  • Failing to provide the right care, in the right place, at the right time
  • Putting patients at risk for potential harm and undesired outcomes
  • Increasing the burden on clinicians and contributing to moral distress

Diversions, long waits, and delays in the emergency department (ED) are a hospital-wide issue, not solely an ED issue. Waits are often the result of ED beds being occupied by patients waiting for admission to the hospital (known as boarding). Lack of inpatient capacity can also result in patients boarding in the post-anesthesia care unit (PACU) and being managed in "off-service" units.

While many hospital leaders are aware of the problem, they may struggle with developing the strategies needed to address these complex issues.

Agenda

This program consists of 10 live virtual sessions from 10:00 AM – 1:30 PM ET.

Participants must attend all live sessions to receive a certificate of completion and credits for this program.

Emergency departments (EDs) frequently have inadequate capacity to evaluate and treat individuals with mental health conditions, thereby over-burdening staff, raising costs, and leading to poor outcomes and experience of care. The Institute for Healthcare Improvement (IHI), in partnership with Well Being Trust, ran an 18-month Learning Community in which multi-disciplinary teams from eight hospitals tested a change package and associated measures focused on the care experience for individuals with mental health or substance use disorders. This online course with coaching contains the learnings from these eight pilot hospitals, from improving health outcomes and patient and staff experience to strategies for reducing avoidable ED re-visits. Through 5 interactive sessions, with additional all learner calls and 1:1 coaching, participants will gain practical guidance, tools, and resources to begin this transformative work within their own organizations.
Recognize the importance of building a trauma-informed culture in the ED through this 5-session online course with coaching.
What You Will Learn
What You Will Learn
